Planning Board Forms

Forms to be filled out and submitted to the Town in order to be placed on an upcoming Planning Board agenda. 

The Board requires submissions 20 days prior to a scheduled Planning Board meeting. 

Please file all submissions with the Town Clerk. 

 


Conditional Use Permit Forms

Inquiry Forms are recommended to get initial feedback from the Planning Board regarding use or feasibility of a potential development. This is an opportunity to understand the review process with the Planning Board. 

 

Conditional Use Permit Forms are to be filed with the Town Clerks Office 20 days in advance of a scheduled Planning Board meeting date. Prior to or at the time of filing with the Town, applicants must mail Abutters Notices (see page 5 of application form).  The application fee must also be paid at the time of submission to the Town Clerks Office.

Minor Subdivision

$400 per lot/ dwelling unit proposed plus $75 per lot/dwelling for escrow, plus additional escrow as needed

 

 

Major Preliminary Subdivision

$400 per lot/ dwelling unit proposed plus $350 in escrow for public hearing plus $45 per lot/dwelling proposed for peer review escrow, plus additional escrow as needed

 

 

Major Final Subdivision

$300 per lot/ dwelling unit proposed plus $350 in escrow for public hearing plus additional escrow as needed

 

 

Revisions to Approved Subdivision Plans

$100 base fee plus $300 per lot/ dwelling unit proposed plus $350 in escrow for public hearing plus additional escrow as needed
